Do I need Botox or filler?

One relaxes muscle, the other adds structure. A thirty second test in a mirror usually tells you which problem you have.

Botox relaxes a muscle. Filler adds volume or structure. So the question is not which is better, it is which problem you have. Lines that appear when you move your face are a muscle problem and belong to Botox. Hollows, flatness and loss of structure that are there when your face is still belong to filler.

The mirror test

Look at the thing you dislike with your face completely relaxed. Then make the expression that produces it.

  • It disappears at rest. That is a dynamic line, caused by muscle movement. Neurotoxin is the tool.
  • It is there at rest and does not change when you move. That is volume, shadow or skin change. Filler, or something other than an injectable, is the tool.
  • It is there at rest and gets deeper when you move. Very common, and the answer is usually both, in sequence rather than at once.

What each one is actually good at

The vertical lines between the brows, horizontal forehead lines, crow’s feet, and a set of less obvious uses: jaw tension and width through the masseter, and brow position, which is a balance problem between muscles rather than a volume one.

Filler, where it earns its reputation

Structure along the jaw and chin, midface support, lips, and, selectively,the under eye, which has the narrowest candidacy of anything on this list.

The sequencing point most people are not told. When both are indicated, treating the muscle first and reassessing is usually the better order. Relaxing a muscle changes how the overlying tissue sits, which changes how much filler the area needs, sometimes down to none. Doing it the other way round means you may pay for volume that the neurotoxin would have made unnecessary.

When the answer is neither

Skin texture, fine crepey lines, pigment and laxity are not muscle problems and not volume problems. Injecting either product into them produces a result that disappoints at two weeks. Loose skin in particular is a surgical conversation, and a good consultation says so rather than proposing syringes.

The practical difference in commitment

  • Neurotoxin wears off completely. If you dislike it you wait it out, generally on the order of months, and nothing is permanent.
  • Hyaluronic acid filler is reversible. It can be dissolved deliberately, which is a genuine safety net and a reason to be cautious about products that cannot be.
  • Neither one makes you worse if you stop. Your face resumes the ageing it was already doing. The claim that stopping accelerates anything is marketing.

What to bring to the consultation

Not a product request. Bring the thing you dislike, and whether it is there when your face is still. That single observation does most of the work, and it keeps the conversation on your face rather than on a menu.
